Manutenção em Ritmo Sinusal
No Brasil existe um número limitado de medicações disponíveis para manutenção em ritmo sinusal no paciente com fibrilação atrial. Dentre elas, podemos citar a propafenona, sotalol e amiodarona. A propafenona é uma medicação que deve ser usado em coração sem alteração estrutural e além de utilizada para manutenção do ritmo sinusal também pode ser utilizada na reversão aguda da arritmia. O sotalol é bastante útil na manutenção do ritmo. Deve-se ter cautela nos primeiros dias de uso pela possibilidade do aumento do intervalo QT, podendo levar a torsade de pointes. Deve ser evitado em pacientes com disfunção ventricular. A amiodarona é a medicação de escolha no paciente que tem alteração estrutural importante e também tem utilidade na reversão aguda da arritmia.
Controle da Frequência cardíaca
Os betabloqueadores são as medicações mais utilizadas nessa estratégia. Dentre eles podemos citar os seletivos beta 1 (atenolol, bisoprolol, esmolol e metoprolol), não seletivos (propranolol e naldolol) e não seletivos com efeito no bloqueio alfa-adrenérgico (caverdilol e labetalol).
Podemos utilizar também os bloqueadores de cálcio não diidropiridinicos. Os exemplos seriam o Verapamil e Diltiazem. Devem ser evitados na disfunção ventricular.
A digoxina já foi muito utilizada no passado porém cada vez mais está em desuso para pacientes com fibrilação atrial. Em 2017 uma subanalise do estudo ARISTOTLE mostrou associação de aumento da mortalidade em pacientes com fibrilação atrial e uso de digoxina.
